A Bariatric Psychologist’s Job Is Not to Fail You. It Is to Get You Ready.
If your surgeon just told you that you need a psychological evaluation before bariatric surgery, take a breath. The evaluation is not designed to disqualify you. It exists to make sure you walk into surgery prepared, supported, and set up to succeed. At Bariatric Counseling Center in San Antonio, a bariatric psychologist conducts your evaluation inside a program that holds The Joint Commission’s Gold Seal of Approval and does nothing but this work: the psychology of eating, weight, and bariatric surgery.
What a Bariatric Psychologist Evaluates, and Why Surgeons Require It
Nearly every bariatric surgery program, and most insurance plans, require a pre-surgical psychological evaluation. Surgery changes your stomach; it does not change the habits, emotions, and history that shaped your eating. Surgeons require the evaluation because untreated binge eating, unmanaged depression, or unrealistic expectations are among the strongest predictors of poor surgical outcomes.
At BCC, your evaluation with a bariatric psychologist typically covers:
- Your weight and dieting history, and your relationship with food today
- Screening for binge eating disorder, compulsive overeating, and emotional eating
- Mood, anxiety, trauma history, and current stressors
- Substance use screening, since risk can shift after surgery
- Your understanding of the procedure, the required lifestyle changes, and your support system
- Readiness, motivation, and expectations for life after surgery
You will get straight answers, and so will your surgeon. Most clients leave with a clear report and practical recommendations. When the evaluation surfaces something that needs work first, that is not a rejection; it is the evaluation doing its job, and BCC can provide the treatment that gets you to surgery genuinely ready.
More Than an Evaluation: A Full Clinical Team Behind It
The evaluation matters, but what sets BCC apart is what stands behind it. We are the only program of its kind in Texas: a Joint Commission accredited intensive outpatient program built entirely around bariatric and disordered eating psychology.
Our team includes Clinical Psychologists and Licensed Professional Counselors, Registered Dietitians, a Registered Nurse, and a Professional Chef. For clients who need treatment before or after surgery, our IOP runs about three months and roughly 30 sessions, two to three times per week, with morning (11am to 2pm, Tue-Fri), evening (5:30 to 8:30pm, Mon-Thu), and Saturday (10am to 1pm) tracks. Treatment draws on DBT, RO-DBT, CBT, CBT-E, and ACT, delivered through individual, group, and family therapy, plus mindful eating training, cooking classes, and guided grocery store tours.
That depth matters after surgery too. Many patients hit the hard months when old eating patterns resurface, weight regain starts, or food loses its role as a coping tool and nothing has replaced it. A bariatric psychologist who knows your history, inside a program built for exactly this, is a very different resource than a general therapist learning bariatrics on your time.
Insurance, Scheduling, and Telehealth Across Texas
BCC is located in the Medical Center area of San Antonio, close to the surgical practices many of our clients come from, and we serve adults across all of Texas by telehealth.
We accept most major insurance plans, including Blue Cross Blue Shield, Aetna, Cigna, UnitedHealthcare, Tricare, Humana, UMR, Carelon, Oscar, Imagine Health, Sana, MultiPlan, Meritain Health, ComPsych, Curative, 90 Degree Benefits, and Healthcare Highways. CareCredit is accepted. If your insurer denies coverage, BCC files appeals on your behalf at no cost.
One more practical advantage: surgical teams across San Antonio’s Medical Center already know BCC’s evaluation reports — they have been reading them for years. We schedule evaluations quickly because your surgery date depends on it, and we communicate directly with your surgical team so nothing stalls between our report and your approval.
Frequently Asked Questions
What does a bariatric psychologist do?
A bariatric psychologist specializes in the mental and behavioral side of weight and bariatric surgery: conducting pre-surgical evaluations, treating binge and emotional eating, and supporting patients through the psychological adjustments after surgery.
How long does a bariatric psychological evaluation take?
Plan for a clinical interview plus standardized questionnaires, typically completed in one to two visits. BCC then prepares the report your surgeon and insurance require, and we keep turnaround tight because we know your surgery date depends on it.
Can you fail a bariatric psych evaluation?
It is not pass-fail. Most people are cleared, some with recommendations. If something needs attention first, such as untreated binge eating or unstable depression, the evaluation identifies it and BCC can treat it, which protects your surgical outcome rather than blocking it.
Does insurance cover the psychological evaluation for bariatric surgery?
Usually yes, since most insurers require the evaluation as part of surgical approval. BCC accepts the major carriers listed above, verifies your benefits up front, and files appeals at no cost if needed.
Can I do my bariatric psych eval by telehealth in Texas?
Yes. BCC provides evaluations and treatment by telehealth to adults anywhere in Texas, with the same clinicians who see clients in our San Antonio Medical Center office. Confirm with your surgical program that a telehealth evaluation meets its requirements; most accept it.
What if I already had surgery and I am struggling now?
You are exactly who our program was built for. Regain, returning binge urges, and post-surgery depression are common and treatable, and BCC’s full clinical team works with post-operative patients every day.
